The enquiry form collects a contact name, the student's first name and year level, an email address, an optional phone number, a preference for in-person or online support, the general areas of support you are interested in, and any message you choose to write.
Please do not send medical records, diagnostic reports or other sensitive health information through the website form. If any of that information becomes relevant, it can be discussed directly.
How the information is used
Enquiry details are used only to reply to you and to arrange tutoring or learning support. They are not sold and are not shared with third parties for marketing.
You can ask for your enquiry details to be corrected or deleted at any time by replying to any message from ClearPath Learning.
